Ear surgery is common in Pakistan, whether it is tympanoplasty (eardrum repair), mastoidectomy (infection removal), or stapedectomy (stapes bone replacement). Many patients expect their hearing to be fully restored after surgery, but in some cases, residual hearing loss remains. This is where hearing aids become an essential part of the recovery journey.
Common Ear Surgeries in Pakistan
Tympanoplasty (Eardrum Repair)
This is the most common ear surgery in Pakistan, often performed to repair a perforated eardrum caused by chronic ear infections (CSOM). While tympanoplasty repairs the eardrum, it does not always fully restore hearing, especially if the middle ear bones (ossicles) were also damaged by infection. Many patients still have 20-40 dB of residual hearing loss after surgery.
Mastoidectomy
Performed to remove infected mastoid bone tissue, usually due to cholesteatoma or chronic mastoiditis. This surgery often involves significant modification to the ear canal structure, which affects how hearing aids fit. Modified radical mastoidectomy can leave patients with a large cavity that requires specially fitted ear molds.
Stapedectomy
This surgery replaces the immobile stapes bone in otosclerosis patients. While success rates are high (85-95%), some patients still have residual conductive hearing loss or develop sensorineural hearing loss over time. Hearing aids may be needed if surgery does not fully restore hearing.
When Can You Wear a Hearing Aid After Surgery?
The timeline for wearing hearing aids varies by surgery type and individual healing:
- Tympanoplasty: Typically 6-8 weeks after surgery, once the graft has fully healed and your ENT surgeon confirms the ear canal is dry and healthy.
- Mastoidectomy: Usually 8-12 weeks, depending on the extent of surgery and how the cavity heals. Open cavity mastoidectomy may require a longer wait.
- Stapedectomy: Generally 4-6 weeks, but your surgeon will advise based on your healing progress. Avoid any pressure in the ear canal during early recovery.
- Myringotomy with tubes: Hearing aids can usually be worn with ventilation tubes in place, but special open-fit ear molds are recommended to allow tube drainage.
Choosing the Right Hearing Aid After Surgery
Post-surgical ears often have unique requirements that affect hearing aid selection:
For Post-Tympanoplasty Patients
A standard BTE hearing aid with a custom ear mold works well for most tympanoplasty patients. The ear mold must be made after surgery, once the ear canal has fully healed and stabilized. Never reuse an old ear mold as the canal shape may have changed after surgery.
For Post-Mastoidectomy Patients
Mastoidectomy patients, especially those with open cavities, need special consideration. A BTE hearing aid with a carefully fitted soft silicone ear mold is usually best. The mold must be designed to avoid irritating the cavity walls. In some cases, a bone conduction hearing aid may be recommended if the ear canal cannot tolerate conventional hearing aids.
For Post-Stapedectomy Patients
If hearing loss persists after stapedectomy, a RIC hearing aid is often ideal because of its light, open fit that minimizes pressure in the ear canal. This is important for ears with surgical prostheses.
Post-Surgical Hearing Aid Fitting Process
Here is what to expect when getting fitted for a hearing aid after ear surgery:
- Step 1 - ENT clearance: Bring a letter from your surgeon confirming you are cleared for hearing aid use.
- Step 2 - Hearing test: We conduct a fresh PTA hearing test to assess your current hearing levels post-surgery. Your pre-surgical audiogram may no longer be accurate.
- Step 3 - Ear impression: If a custom mold is needed, we take an impression of your ear canal.
- Step 4 - Fitting: The hearing aid is programmed based on your new audiogram and fitted with the custom mold.
- Step 5 - Follow-up: We schedule follow-up visits at 1 week, 1 month, and 3 months to ensure comfort and optimal performance.
Tips for Post-Surgical Hearing Aid Users
- Keep your ear and hearing aid absolutely dry to prevent infection in the surgical site
- Clean the ear mold daily with antiseptic wipes
- Start wearing the hearing aid for short periods and gradually increase wearing time
- Report any pain, discharge, or unusual sensations to your ENT surgeon immediately
- Attend all follow-up appointments with both your surgeon and audiologist
